To commission a Warm Homes single point of contact (SPoC) health and housing referral service which vulnerable people living cold and fuel poor households can be referred by themselves or professionals for an integrated assessment and tailored onward interventions. The aim is to provide a single access point to timely, effective, and appropriate support that mitigates and minimises the risks associated with cold conditions and winter weather on the health and wellbeing of our most vulnerable groups.
